If you would like your child to ride in church sponsored transportation to/from camp, please complete this form. The cost per child is $30 to cover fuel and lunch on the way to and from camp. Seats are limited and available on a first come, first served basis. A liability/consent form will need to be completed prior to departing for camp.

In addition to the van driver, we need another chaperone to help the campers get checked into camp on Monday and to pick them up from camp on the last day. Please let us know your availability by checking the appropriate box on this form.

Sibling Discount: If more than one child from the SAME family is going to the SAME camp, the transportation cost per subsequent child is only $20.
